| Contest Rules: |
 All referrals must be new candidates to Attentive Healthcare. |
| Candidates must accept a job or assignment in order to qualify. |
| Candidate must complete the first 40 days in order for referral to be valid. |
| A minimum of 4 placed candidates are needed to qualify for the trip. If minimum is not reached, a referral bonus will be given per candidate. Bonus will be processed at the end of the candidate’s first 90 day assignment or job with Attentive Healthcare. |
| You may not refer yourself. |
| To qualify for the trip, referral must be made between the eligible dates of May 24, 2004 – September 24, 2004. The referral candidate’s assignment must begin by October 20, 2004. |
| Winners will be announced at the end of October. |
| The vacation will include airfare, hotel, and applicable taxes. Not redeemable for cash. |
| Holiday dates are blacked out. |
